There is a significant difference in the percentage payable amounts for In-Network and Out-of-Network Comprehensive Health Benefits (includes medical, mental health, and substance use).  To minimize your Out-of-Pocket expenses, you should use In-Network Providers whenever possible. For 2023, 1095-B Forms are not required to file your taxes.  Therefore, forms will only be mailed upon request.  You can access forms by signing in to the Participant Portal or you can submit a request by contacting our office at any of the following:  1.
