This notice describes changes to the Plan and supplements the 2023 Boilermakers National Health and Welfare Fund Summary Plan Description (SPD). Plan changes are effective as of January 1, 2025, unless otherwise stated, and include the following: Retail pharmacy benefits for Retiree Coverage; An increase to supplemental life insurance of up to $20,000; and An increase to supplemental accidental death and dismemberment (AD&D) benefits of up to $20,000.

There is a significant difference in the percentage payable amounts for In-Network and Out-of-Network Comprehensive Health Benefits (includes medical, mental health, and substance use).  To minimize your Out-of-Pocket expenses, you should use In-Network Providers whenever possible.
