For 2025, 1095-B Forms are not required to file your taxes.  Therefore, forms will only be mailed upon request.  You can access forms by signing in to the Participant Portal or you can submit a request by contacting our office at any of the following:  BNF@Wilson-McShane.com, call 866-342-6555 – option 6, or by mail at PO Box 909700 Kansas City, MO 64190-9700.  If you had coverage through United Healthcare (UHC) in 2025, UHC will provide forms to you.  Contact UHC at 877-755-5349 with any questions.
