Coming soon to your mailboxes!   This notice describes changes to the Plan and supplements the 2023 Summary Plan Description (SPD).  Plan changes are effective as of January 1, 2026, unless otherwise stated.  Included are an increase to the annual vision benefit limit to $500 and Delta Dental’s Special Health Care Needs Program.

Hazelden Betty Ford’s Connection program was added to the Plan effective July 1, 2025, for Active and Retiree Coverage.  The Connection program provides virtual tiered support, coaching, and monitoring as an additional tool for individuals undergoing substance use treatment.  After Plan Deductible, benefits are paid at 100%.
