For 2024, 1095-B Forms are not required to file your taxes.  Therefore, forms will only be mailed upon request.  You can access forms by signing into the Participant Portal or you can submit a request by contacting our office at any of the following:

If you had coverage through United Healthcare (UHC) in 2024, UHC will provide forms to you.  Contact UHC at 877-755-5349 with any questions.
