SUMMARY OF MATERIAL MODIFICATIONS (SMM) NOTICE OF AMENDMENT 5

This notice describes changes to the Plan and supplements the 2023 Boilermakers National Health and Welfare Fund Summary Plan Description (SPD). Plan changes are effective as of January 1, 2025, unless otherwise stated, and include the following:
- Retail pharmacy benefits for Retiree Coverage;
- An increase to supplemental life insurance of up to $20,000; and
- An increase to supplemental accidental death and dismemberment (AD&D) benefits of up to $20,000.
